Returning clients of Clark Family Eyecare will require a Unique Identifier Number (UIN), this will be sent out with any eye examination reminder notifications. The UIN will enable our secure systems to link your client record with the appointment booked online. If you do not have this, please call the practice or pop us a message below.
Please do not sign in as a new customer if you have used our services previously.
