We collect information about you when you register with us, book an appointment or place an order for products or services. We also collect information when you voluntarily provide feedback and participate in competitions. We may collect the following information such as name, title, date of birth, contact information including address, telephone numbers, postcode and email address, and demographic information such as preferences and interests.
How do we use this information?

Clark Family Eyecare keep records of our information about you as a mixture of paper and/or computer records to provide the products and services that you request and for internal record keeping.
You are entitled to a copy of your records. If you wish to see your records, please contact us and we will respond as quickly as possible and in accordance with the current data policy standards.
Text Messaging Service
If we have received your consent, we will send confirmation of your booked appointments with us to your mobile telephone.

Email Communication
We will send emails to you for feedback regarding your recent visit to the practice. Occasionally, we may send you our practice newsletter and communications relating to our business, products or services which we think may be of interest to you. If you no longer wish to receive this information from us, please email us on ??@clarkeyecare.co.uk with your request.
Disclosure
We will not disclose your personal information to third parties unless we have your signed permission or are required by law to do so.
If you are an NHS patient, the NHS may ask to see the portion of your record that relates to NHS services provided. Such information will only be given to the NHS in strictest confidence.
Security
We keep and use information in line with the current law. Paper records are stored in a password control locked cupboard and all computers are password protected. Backups of computer records are encrypted and stored off site to safeguard and secure the information we collect. We also review and update our security measures on a regular basis.
Everyone in the practice is aware of the confidential nature of our information systems and are obliged to respect the confidentiality of any personal information held by us.
If you believe that any information we are holding on you is incorrect or incomplete, please contact us as soon as possible and we will amend any information found to be incorrect.
